Reprocess Files Re-read every file, regenerate .md summaries, and rebuild _INDEX.md files. Does NOT move anything.

Useful when you've upgraded the LLM, changed summary prompts, or want to refresh stale descriptions. This walks every folder under DIR, downloads each file again, re-summarises it with the LLM, replaces the companion .md, and rebuilds _INDEX.md bottom-up. Media files are skipped. File locations are never changed.
